Model Notices of Creditable Coverage

Federal law requires you to provide Part D-eligible individuals who are covered under your prescription drug coverage with notice of creditable coverage on an annual basis. To assist you in this requirement, visit the CMS website to view or print the Creditable Coverage Guidance and Model Disclosure Notices. The notices are available in English and Spanish; there are creditable and non-creditable versions for your use.
