Our Drug Formulary
The Blue Cross and Blue Shield of Nebraska drug formulary is a list of medications included in most
prescription drug benefits. Formulary drugs are medications recommended by the
Blue Cross and Blue Shield of Nebraska Pharmacy and Therapeutics Committee.
These drugs are selected based on a quality evaluation of safety, effectiveness,
unique qualities and cost. The Committee meets quarterly to update the
formulary.
Depending on your coverage,you may pay lower copay amounts when your prescriptions are filled using
formulary medications. Please refer to your Certificate of Coverage and/or
Schedule of Benefits to determine the specific coverage and prescription drug
benefits available under your plan. If you have additional questions or would
like a printed copy of the drug formulary, contact Blue Cross and Blue Shield of
Nebraska Customer Service Center at the number on the back of your ID card.

formulary does not ensure coverage.
